Search for a biological age test and page one hands you a biological age calculator, then another. The top organic result belongs to a dental practice, another is a supplement company's free biological age test, and a video titled "Test Your Biological Age For $0" has nearly a million views. Almost nobody is selling you an at home biological age test on evidence.

Almost none of them tell you how reproducible any of this is. That is the number this page leads with, because it decides whether the rest is worth your money.

What is a biological age test?

A biological age test estimates how old your body appears to be functionally, as distinct from how many years you have been alive. That gap, biological age versus chronological age, is the whole proposition. Consumer tests use one of three methods: DNA methylation patterns read from a blood or saliva sample, algorithms built on routine blood biomarkers, or physical function measures such as grip strength and the sitting-rising test.

Cleveland Clinic frames biological age as an accumulation of cellular damage over time, and notes in the same piece that it is not clear how reliable these tests are.[5] That caution is well placed and, as you will see below, quantifiable.

One framing point before anything else. Every product here is a wellness product not a diagnostic, not FDA cleared, and none of them replaces standard clinical screening. We unpack the underlying concept separately in what biological age actually means.

How we review these tests

We ask two questions of every product on this page: which published clock or algorithm generates the number, and what its measurement error is. Vendors are rarely eager to answer either.

  • Effect sizes, not adjectives. A hazard ratio with a confidence interval beats "clinically validated" every time.
  • Published instrument, named. If a score cannot be traced to a published clock, we say so.
  • No kit of our own. We sell no test and take no vendor commission.
  • Dated corrections. When the evidence changes, this page changes and says when.

We have not run these kits ourselves and do not pretend otherwise. What follows reviews published validation data and vendors' own public claims.

How do I test my biological age?

Start with two free tests that take about ten minutes between them and have real mortality data behind them. Do these before you spend anything, because they set a baseline you can repeat for free forever.

Write down both numbers with today's date. That record is worth more than a single expensive score with nothing to compare it to.

Test 1: the sitting-rising test

Sit down on a bare floor, cross-legged, then stand back up. Score five points for sitting and five for rising, out of 10 total.

Deduct one point each time you use a hand, a knee, a forearm or the side of your leg for support. Deduct half a point for a visible loss of balance.

That hazard ratio 5.44 comes from 2,002 adults aged 51 to 80, followed for a median of 6.3 years, with 159 deaths recorded. Each single point gained on the score was associated with a 21% improvement in survival.[2]

The test is a proxy for the bundle of strength, balance, flexibility and body composition that decides whether an older adult gets up off a floor unaided. It is not a diagnosis and it is not destiny.

Test 2: grip strength

Grip strength needs a hand dynamometer, which costs under $30. Take the best of three squeezes on each hand and record the higher hand.

In the PURE study, conducted across 17 countries, the figure was a hazard ratio 1.16 per 5 kg reduction in grip strength for all-cause mortality, with a 95% confidence interval of 1.13 to 1.20.[3] Cardiovascular and non-cardiovascular mortality showed the same 1.17 figure.

The detail worth sitting with: in that dataset, grip strength predicted death more strongly than systolic blood pressure did.

Test 3: the numbers you already have

Resting heart rate, blood pressure, waist-to-height ratio and a standard lipid and glucose panel are the raw inputs of the entire blood-biomarker family of biological age algorithms.

This is not an argument against paying for a test. It is an argument for knowing what each row buys you before you choose.

What epigenetic clocks actually measure

An epigenetic clock reads methylation levels at selected CpG sites in your DNA and feeds them into a trained algorithm. The output is an age estimate, and which age it estimates depends entirely on what the algorithm was trained against.

First-generation clocks, Horvath and Hannum among them, were trained to predict chronological age. Second-generation clocks such as PhenoAge and GrimAge were trained against health outcomes instead. DunedinPACE takes a different approach again and estimates pace of aging rather than an age, and the Horvath clock remains the reference point everything else is compared against.

They are not interchangeable, and a vendor that will not name its clock is withholding the one fact you need to interpret the result. The broader biology sits in the hallmarks of aging.

A clock that matches your birthday is not detecting health

This trips up almost everyone. If a first-generation clock returns an epigenetic age within a year of your chronological age, that is the model hitting its training target, not evidence that you are aging normally.

The clocks designed to say something about health were trained against outcomes, not against your date of birth. Ask which one produced your number before you read anything into it.

How accurate is a biological age test?

Less reproducible than the price tag implies. A 2022 analysis in Nature Aging found that technical noise produced deviations of up to 9 years between replicate samples across six prominent epigenetic clocks.[1]

Read that carefully. The same DNA, split and run twice, with no biology changing in between, could return two numbers nine years apart.

The same paper supplies the fix. A principal component clock, recalculated from the same CpG data, brought most technical replicates into agreement within about 1 year and improved stability in longitudinal tracking. Those deviations up to 9 years are a property of the standard versions, not of the underlying biology.

Not one vendor on page one of this search publishes its own test-retest data. That absence is the single most useful thing to notice about the category.

What counts as a real change

If a standard clock can wobble by years on identical samples, a two-year improvement after six months of effort may be measurement error wearing a success story.

Before you accept any change as real, ask the provider for its measurement error on split samples. If you intend to track over time, prefer a principal-component or pace-of-aging output, and leave enough interval that biology can outrun the assay.

What is the most accurate biological age test available?

There is no defensible single answer today, and the honest sources say so. The Max Planck Institute for Biology of Ageing states that biological age cannot at present be measured accurately using a single test.[4]

Accuracy is also the wrong question for most buyers. Accurate against what: your chronological age, your mortality risk, or your rate of change?

If you are tracking an intervention, reliability beats accuracy and they are different properties. A slightly biased measure that repeats consistently is more useful than an unbiased one that scatters by years.

Is there a free online biological age test available?

Yes, several, and they are questionnaires rather than measurements. A survey-based calculator converts self-reported behavior, height, weight and habits into an age estimate.

That is genuinely useful for motivation and largely useless for judging whether something you did worked, because nothing physical was measured. It reflects what you told it and inherits every optimism in your self-report.

The two physical tests earlier on this page cost nothing and measure something. If you only do one free thing today, do the sitting-rising test.

How much does a biological age test cost?

Consumer DNA methylation kits generally sit in the low hundreds of dollars per test, and most providers push a retesting subscription that multiplies the annual figure.

Survey calculators are free. Broad biomarker panels vary widely and are usually bought for the panel rather than for the age number attached to it.

Insurance does not cover wellness testing. HSA and FSA eligibility depends on your plan, and prices move, so verify before ordering rather than trusting a figure on any article including this one.

Questions to ask before you buy a kit

  1. Which published clock generates my number, by name?
  2. What is your test-retest reliability on split samples, in years?
  3. Is the sample blood or saliva, and which was the clock validated on?
  4. What size of change, over what interval, would you call real?
  5. What exactly does the organ-level breakdown rest on, if you offer one?

A provider that answers all five is worth paying. A provider that answers none is selling a number, not a measurement.

What actually moves the number

Smoking status carries the largest single signal in outcome-trained clocks, which is worth knowing before you optimize anything more exotic.

Cardiorespiratory fitness, body composition and sleep move biomarker-based scores. Grip strength and sitting-rising scores respond to training within months, which is part of why they are satisfying to retest.

Using a test to judge an intervention

Baseline first, before you start. Testing after the fact produces a number with nothing to compare it against, and it is the most common expensive mistake in this category.

Then hold everything constant: same provider, same clock, same sample type, similar time of year, and a retest interval long enough to matter. This matters most for people measuring a specific compound, and rapamycin's own evidence base is a good example of why a single wobbly readout should not decide anything.

Who should not bother

  • Anyone who would not change a thing based on the result. The number is only worth its price if it moves a decision.
  • Anyone planning to retest monthly. You will be reading assay noise as progress.
  • Anyone substituting it for clinical screening. Blood pressure, lipids, glucose and age-appropriate cancer screening are not optional and these tests are.
  • Anyone who has not done the two free tests yet. Ten minutes, no cost, published hazard ratios.

Should you buy one? An honest decision aid

  • If you want a mortality-linked number today, free, do the sitting-rising test and record it.[2]
  • If you are measuring an intervention, buy reliability: a principal-component or pace-of-aging output, and a provider that will state its measurement error.[1]
  • If you want organ-level detail, ask for the validation data behind the organ scores before paying for them.
  • If you want motivation, a free survey calculator is fine as long as you know it is a questionnaire.

The field will get better at this. Principal-component clocks already show what a reliable version looks like, and pace-of-aging measures answer a more useful question than a single age. Until vendors publish their own reproducibility figures, the free tests remain the best evidence-to-dollar ratio in consumer longevity measurement.
