What is rapamycin? One molecule with five different jobs
Rapamycin is an antifungal, an immunosuppressant, a stent coating, a cancer drug and a longevity candidate. It is the same molecule in all five roles, and most confusion about it comes from reading about one job while thinking about another.
Where it came from
A 1960s expedition collected soil on Rapa Nui, the island most people know as Easter Island. A bacterium in that sample, Streptomyces hygroscopicus, produced a compound with strong antifungal activity, and researchers named it after the island.[1] The antifungal program went nowhere commercially, because the same molecule turned out to suppress the immune system, which made it useless as an anti-infective and extremely valuable as a transplant drug.
That accident of history is why the drug has two names. Rapamycin is the original laboratory name. Sirolimus is the international nonproprietary name assigned at approval, and Rapamune was the original brand. When a page compares rapamycin and sirolimus as though they might differ, it is comparing a molecule with itself.
What is rapamycin approved to do
Strong human evidence The approved indications are narrow and well established. It prevents rejection in kidney transplant recipients. It coats coronary stents, where local mTOR inhibition stops the vessel wall from re-thickening around the implant. It treats lymphangioleiomyomatosis, a rare progressive lung disease. Its chemically modified relatives, everolimus and temsirolimus, carry approvals in oncology.[2]
Aging is not on that list, and there is no application pending to put it there. Prescribing it for longevity is off-label, which is legal and common practice for approved drugs but places the entire risk judgment on the prescriber and the patient.
How it works, in one paragraph
Inside the cell the drug binds a small protein called FKBP12. The resulting complex latches onto mTOR complex 1, which is the master switch that reads nutrient availability and decides whether the cell should be building or maintaining.[3] Inhibited, mTORC1 slows protein synthesis and cell growth and lets autophagy, the cell's recycling program, run harder.
There is a second complex, mTORC2, which governs different things including insulin signaling and is far less sensitive to brief exposure. That asymmetry is the entire rationale for weekly rather than daily dosing: hit mTORC1, leave mTORC2 largely alone. Why longevity researchers care
In 2009 the Interventions Testing Program, a multi-site program specifically designed to weed out irreproducible lifespan claims, reported that mice given rapamycin starting at 600 days of age lived measurably longer.[4] Starting at 600 days matters: these were not young mice given a lifelong intervention, they were already old.
Nothing else in the field has that record. It extends lifespan in yeast, worms, flies and mice, and the mouse result has been replicated across sites and strains.[5] A systematic review of human and rapalog trials found the human evidence far thinner: real biological effects, no lifespan endpoint anywhere.[6]
Is rapamycin the same as metformin?
No. They get discussed together because both are repurposed prescription drugs with longevity interest, and the similarity stops there. Metformin is a decades-old diabetes medicine, cheap, with enormous safety data, acting largely through AMPK and mitochondrial signaling. Rapamycin is an immunosuppressant acting directly on mTORC1.
The evidence runs in opposite directions too. Rapamycin extends mouse lifespan consistently; metformin has repeatedly failed to in the same testing program, and its human longevity case rests on observational data awaiting a proper trial. We compare them properly in rapamycin versus metformin.
Can you get rapamycin naturally?
No food contains it. It is a bacterial secondary metabolite, not a plant compound, and no dietary source exists at any meaningful quantity. The persistent search for foods containing it is answered simply: there are none.
What does exist is a group of compounds that touch the same pathway from a distance, mostly spermidine and various polyphenols, sold as natural mTOR inhibitors. Can you buy rapamycin over the counter?
Not legally in the United States. It is prescription-only, which means a licensed clinician and a dispensing pharmacy. Anything sold without a prescription and marketed as a rapamycin supplement is a different substance wearing the name.
The practical route for off-label longevity use is a physician willing to prescribe it, generic sirolimus rather than brand Rapamune, and periodic monitoring. Costs and access routes are covered in our page on what it costs and how people obtain it.
What the honest summary is
Early / mixed evidence A recent review in Aging concluded that the clinical picture in healthy adults remains insufficient to support routine off-label use.[7] A 2025 review in Frontiers in Aging laid out the pros and cons and reached a similar place: mechanistically compelling, clinically premature.[8]
Rapamycin, in my view, still our best shot on goal.
So the answer to what is rapamycin depends on which of its five jobs you mean. As a transplant drug it is established and unremarkable. As a longevity intervention it is the single most convincing animal result in the field attached to the thinnest human evidence base, taken by a growing number of people who are, collectively, the trial. What is rapamycin used for?

The unapproved use is off-label weekly dosing by healthy adults hoping to slow aging.
Is rapamycin an antibiotic?
By origin, yes. It was isolated from a soil bacterium and first characterized as an antifungal agent. Its immunosuppressive properties were discovered afterwards and turned out to be commercially far more important, which is why nobody uses it as an antifungal today.
Is rapamycin the same as sirolimus?
Identical molecule, two names. Rapamycin is the original research name, taken from Rapa Nui. Sirolimus is the international nonproprietary name it received on approval. Rapamune is the original brand. Everolimus and temsirolimus are different molecules, chemically modified relatives known as rapalogs.
Is rapamycin the same as metformin?
No, and they are not close. Metformin is a cheap diabetes drug that works largely through AMPK and mitochondrial signaling. Rapamycin is an immunosuppressant that inhibits mTORC1 directly. Rapamycin extends lifespan reliably in mice, metformin does not, and metformin has decades of human safety data that rapamycin lacks at longevity doses.
Can you get rapamycin naturally?
No. It is produced by a soil bacterium, not by any plant or food, and there is no dietary source. Anything sold as a natural rapamycin is a different compound entirely, usually spermidine or a polyphenol, marketed on a mechanistic resemblance rather than an equivalence.

The second complex, mTORC2, is much less sensitive to short exposures, which is the biological reason intermittent dosing exists.
Can you take rapamycin for anti-aging?
People do, off-label and by prescription. What supports it is the animal data: it extends lifespan in every model tested, including mice started late in life. What does not yet exist is any human trial showing a lifespan or healthspan benefit, or any validated dose.
What is the downside of rapamycin?
It is an immunosuppressant with a real adverse event profile, including mouth ulcers, lipid changes, blood count changes and infection risk, all dose-dependent. The larger downside is uncertainty: no long-term safety data in healthy people and no agreed dose.
Can rapamycin reverse gray hair?
There is no controlled human trial showing that. Hair color changes appear in animal work and in anecdotal reports from people taking it, which is the weakest evidence category available.
Where does rapamycin come from?
From Streptomyces hygroscopicus, a bacterium in a soil sample collected on Easter Island in the 1960s. The island's Polynesian name, Rapa Nui, gave the compound its name.
Is rapamycin FDA approved?
Yes, for transplant rejection prevention, for lymphangioleiomyomatosis and in stent coatings. It is not approved for aging, longevity or healthspan, and prescribing it for those purposes is off-label use of an approved drug rather than use of an unapproved one.
How long has rapamycin been studied?
Since the early 1970s as a compound, since 1999 as an approved drug, and since 2009 as a longevity candidate, when the Interventions Testing Program reported lifespan extension in mice given it late in life.
What does mTOR stand for?
Mechanistic target of rapamycin, formerly mammalian target of rapamycin. The pathway is named after the drug because the drug was the tool that led researchers to it, not the other way around.
