NAD side effects: two completely different risk profiles
Search results merge capsules and infusions into one list. They are different substances by different routes with different risks, and most of the pages describing them sell one of the two.
Why the route changes everything
The phrase covers at least four different things: nicotinamide riboside, nicotinamide mononucleotide, plain niacin or nicotinamide, and intravenous NAD itself. They have different tolerability profiles, different doses and different regulatory standing, and the search results treat them as one topic.
Separating them is the first useful thing anyone can do here, because the mild reassurance that applies to a capsule does not transfer to an infusion, and the flushing associated with the category belongs to only one of the compounds.
Oral NAD side effects in the trials
Early / mixed evidence Institutional summaries list the possible effects as nausea or stomach discomfort, headache, lightheadedness, diarrhea, muscle cramps and skin irritation, describing the supplements as generally well tolerated.[1] That matches the trial record: a dose-ranging randomized study reported oral dosing safe and well tolerated up to 900 mg daily.[2] Independent evidence summaries reach the same conclusion for nicotinamide riboside.[3]
The limitation is duration rather than severity. Those trials ran weeks to a few months, in modest numbers of participants, against an intended use measured in years.

The compound in the bottle that is not what you think
Many products combine a precursor with plain nicotinamide or niacin, and that matters. High-dose nicotinamide has documented adverse effects, with headache, dizziness and vomiting reported in healthy people at doses of several grams, and reviews raise mechanistic questions about the methyl donors consumed during its metabolism.[4]
Niacin is the source of the flushing the category is known for, and at high doses it is also associated with raised blood sugar and homocysteine.[5] If a product is making you flush noticeably, the ingredient list is usually a better explanation than the headline compound.
Intravenous NAD is a different question
Infusion clinics describe a consistent set of reactions during the drip: flushing and body warmth, chest pressure or tightness, palpitations, lightheadedness, nausea and abdominal cramping. The same sources note that these typically ease when the infusion rate is slowed, which suggests the reaction tracks delivery speed rather than a lasting toxicity.
The more important point is regulatory. Injectable NAD is a compounded preparation, not an FDA-approved finished drug product for energy or aging, and the agency has warned about risks associated with poorly compounded or non-sterile products, including severe chills, fever and vomiting. Coverage of the wider NAD boom has made the same observation about how far the commercial offering has outrun the evidence.[6]
The unknown that applies to all routes
No study has followed anyone taking any of these compounds for years. NAD supports DNA repair and cell proliferation, functions that healthy tissue and existing tumors both use, which is the basis of a persistent theoretical question that no human study has been designed to answer.
That is not a reason for alarm and it is a reason to reject the phrase completely safe. Anyone with a cancer history should discuss supplementation with their oncology team, and nobody should treat weeks of tolerability as a statement about decades.
Whether or not these NAD boosters are effective and safe in humans in the long term is still an open question.
Who has a specific reason for caution
Anyone taking medication for diabetes, since blood sugar effects run in both directions across this family. Anyone pregnant or breastfeeding, on the basis of absent evidence rather than known harm. Anyone with liver disease, given the enzyme elevations occasionally reported at high precursor doses. Anyone considering an infusion, who should ask what is being infused, by whom, at what rate and from which compounding pharmacy.
Weighing NAD side effects against the benefit
The honest frame is not risk in isolation. Oral precursors are mild in the short term, so the question becomes whether a mild risk buys anything. A trial that raised NAD in human skeletal muscle without improving muscle function is the most important answer available, and it is not encouraging.[7]

What is the downside of taking NAD?
For oral precursors, mild and uncommon effects: nausea, stomach upset, headache, and flushing if the product contains niacin. For intravenous NAD, a different and more acute list including chest tightness, warmth and cramping during the drip, plus the risks that come with any compounded injectable.
What happens when you take NAD every day?
Blood NAD precursor levels rise and stay elevated. Trials running weeks to a few months found that unremarkable. Nobody has studied what sustained daily elevation does over years, which is the way people actually use it.
What should you not mix with NAD?
No specific interactions are well documented, which reflects limited study more than a clean bill. The practical cautions are checking whether your product also contains niacin before adding another flushing agent, and telling your prescriber if you take diabetes medication.
Does NAD+ cause weight gain?
There is no evidence that it does. The animal literature points the other way, reporting suppressed age-associated weight gain in mice. Neither direction has been demonstrated in people.
Is NAD similar to Ozempic?
Not remotely. GLP-1 receptor agonists are prescription drugs with large randomized trials showing substantial weight loss and cardiovascular benefit. NAD precursors are supplements that raise a coenzyme, with no comparable outcome data. They appear in the same clinics, which is why the question exists.
Are IV NAD side effects worse?

Is NAD+ really worth taking?
That depends on what you expect. The tolerability of oral precursors is genuinely good. The benefit is unproven, the evidence base is short, and one trial raised NAD in muscle without improving muscle function, which is the result the category has yet to answer.
What would justify stopping?
Persistent gastrointestinal upset, any new unexplained symptom, an abnormal blood result following the start of supplementation, or simply the honest observation that nothing has changed after three months of paying for it.
